Job Description


Functional Duties/Responsibilities:

1. Works directly and in collaboration with other members of the Patient
Centered Medical Home.
2. Acts as firm leader in absence of physician leader.
3. Reports directly to the Head Nurse.
4. Acts as the nurse leader of his / her firm.
5. Works in collaboration with the PCMH team to provide a health care setting
that facilitates partnership between individual patients and their personal
provider and firm members, i.e., nurse, PCAs, social workers, case manager,
administrators.
6. Assess patients based on:
a. Interview of patient and/or significant other.
b. Chart review.
c. Referrals from other health team members and outside agencies.
7. Educates and teaches patients based on diagnosis and need.
8. Initiates appropriate nursing interventions and update electronic medical record as needed.
9. Provide counseling and teaching for patients and their families/significant others who are new or referred by the physician.
10. Provide the appropriate nursing interventions based upon assessment, which will include:
a. Instructing patients and/or family members/significant others regarding treatment, medication, diagnostic procedures and the importance of follow-up visits.
b. Interpreting the patient’s understanding and emotional response to their condition.
d. Conduct / participates in daily administrative / operational huddles with members of the PCMH team.
e. Perform post hospital discharge follow up reminder calls and document outcomes in EMR.
f. Assisting in obtaining and maintaining an accurate list of home medications and ensuring patient’s knowledge and understanding of all medications prescribed.
g. Assisting patients and/or significant others to establish a medication schedule when on multiple medications.
h. Works in collaboration with ancillary staff.
i. Refers patients based on criteria to case / care manager assigned to the Primary Care Medical Home and assist with management of and follow up as requested / needed.
j. Assist with accessing specialty services
k. Making referrals to other agencies as appropriate, i.e., Visiting Nurse Service, Department of Health, Social Services, Home Care, etc.
11. When indicated, schedule appointments for nurse follow-up visits, i.e. anti-coagulant management and compliance, blood pressure check, diabetic self care, self management goals and insulin therapy.
12. Triage patients who present to clinic without appointments, identifying need to:
a. Make appropriate referrals
b. Communicate with physician or other health care members as indicated.
c. Reports findings to firm leader and / or other members of the team
13. Uses the electronic medical record to coordinate care, measure performance, document patient education and enhance communication among the primary care medical team and all its caregivers.
14. Document all observations on one or more of the following tools in the EMR:
a. Initial Nursing Assessment
b. Patient/Family Education
c. Nurse Intake Screening and / or
d. Pain screen and assessment
15. Assist with hospital admission process.
a. Expedite process by communicating with necessary personnel.
b. Support patient and family members.
16. Performs EKG, collects blood specimens when needed
17. Participate in preventive health measures and case findings.
a. Maintain health maintenance status of patient by review of necessary records.
b. Assess for human response to actual or potential health problems at the earliest signs and symptoms and initiate treatment and/or make referrals for appropriate care.
18. Communicate patient care problems/issues via the proper channels.
a. Inform Head Nurse of patients with complicated problems and those who are awaiting admission while acting as the clinical person responsible for the patient’s care
19. Has knowledge of adult immunization schedule and keep abreast of DOH changes.
20. Demonstrates proficiency in the administration of nebulizer and/or metered dose inhalation therapy and documents pre and post assessment.
21. Is able to plant PPD and interpret the results as per guidelines.
22. Ensures a safe and secure environment free of hazards to reduce the risk of injury to patients, visitors, and staff.
23. Acts as a chaperone.
24. Participates in continuing education programs in order to foster growth.
25. Knowledgeable of the glucometer and ISTAT machine use, maintenance and quality control and able to perform glucose and I NR screenings.
26. Check code cart/emergency equipment on a daily basis or as assigned according to protocol.
28. Is aware of disaster plan.
29. Is aware of hazardous waste management program.
30. Aware of clinical alarms in area and responds accordingly.
31. Participates in team building and makes suggestions for improving organization performance.
32. Participates in patient centered scheduling initiatives.
33. Assist in maintaining provider PCMH patient panels.
34. Assists in the development, implementation and data collection of ambulatory care/unit based performance improvement projects.
35. Maintains a level of cultural competence.
36. Aware of the population specific patient management for the area which they work.
37. Understands policy and procedure related to Limited English Proficiency (LEP).
38. Provides excellent customer service.

Minimum Qualifications

For individuals holding an unrestricted registered professional nursing license BEFORE July 1,
2020 in New York State, another state or United States territory:
1. A valid New York State license and current registration to practice as a Registered Professional Nurse
in New York State; and
2. Valid and current certification in Basic Life Support (BLS) through the American Heart
Association (AHA).
For individuals issued a registered professional nursing license on or AFTER July 1, 2020,
based on expected implementation of Chapter 502 of the Laws of 2017 and Chapter 380 of the
Laws of 2018:
1. A valid New York State license and current registration to practice as a Registered Professional
Nurse in New York State; and
a. A Bachelor’s of Science degree in Nursing or higher (or other nursing degree, as identified by NYS
Education Department including Master of Science in Nursing, Doctor of Nursing Practice, Doctor of
Philosophy Degree in Nursing or Doctor of Nursing Science) from an accredited college or
university, registered with the NYSED; or
b. Obtains a Bachelor’s of Science Degree in Nursing or higher, as described in “a” above, within ten
(10) years of the date of initial licensure; and
2. Valid and current certification in Basic Life Support (BLS) through the American Heart Association
(AHA).
